Credits Forms
A writer’s position in the entertainment industry is determined largely by their credits. Their professional status depends on the quality and number of screenplays, teleplays, or stories which bear their names.
Why care about credits?
The Writers Guild of America, East was founded on the notion that writers should get the credit they deserve. That’s why the administration of an accurate and equitable system of determining credits is one of the most important services we perform for writers. To that end, we have provided the credits forms below for your convenience.

Credits Forms
Notice of Tentative Writing Credits (NTWCs)
Before writing credits are finally determined, companies are required to send a completed copy of the NTWC to the Guild, as well as to each participating writer. The NTWC should be sent as soon as possible following the completion of principal photography.
- Theatrical NTWC
- Television NTWC
- High Budget SVOD NTWC (e.g. House of Cards, Orange is the New Black)
- New Media/Digital NTWC
Submitting your NTWC:
The completed NTWC may be delivered to the Guild by email(1), fax, messenger, or any trackable, verifiable means(2).
- Television, High Budget SVOD, and New Media/Digital NTWC should be addressed to the attention of the Administrator of Television & New Media Credits.
- Theatrical NTWC should be addressed to the attention of the Screen Credits Administrator.
Television Collaboration Forms
These forms must be used when a writer-production executive and a writer who is not a production executive collaborate on an episode of an episodic series.
Application to Collaborate | For New Media
This form must be completed prior to the rendition of writing services between both parties and filed with the Guild.
Certificate of Compliance | For New Media
This form must be filed with the Guild after the completion of writing services and before or concurrently with the submission of the Notice of Tentative of Writing Credits for each episode.

Notes:
1 The NTWC may be sent to a participating writer via EMAIL only if the writer’s representative is also given email notice and only if the writer’s personal services contract includes the following: (a) the writer’s express agreement to receive notices by email; (b) the writer’s email address; and (c) the email address of the writer’s designated personal representative, which must be an individual address, not a general company address.
2 Notices may be sent by certified mail, return receipt requested; by a trackable service such as FedEx, UPS or DHL mail; or by personal delivery as above provided. Failure of the addressee to sign or return the receipt on a notice sent via certified mail shall not invalidate the notice.
